Does completing the Interest Form guarantee admission?

No. The Interest Form helps Zen Zone learn about your child and determine whether one of our services may be appropriate. Final admission depends on eligibility, program availability, completion of intake documents, and an assessment.

How will Zen Zone determine whether my child is eligible?

A Zen Zone team member will review the Interest Form and contact you to discuss your child's needs, age, current services, funding, safety needs, and potential program fit. If your child is eligible to continue, you will receive intake documents through SimplePractice and be scheduled for an assessment.

What is OhioRISE?

Accepted Answer

OhioRISE is an Ohio Medicaid managed-care program for youth with complex behavioral-health and multisystem needs. A Child and Adolescent Needs and Strengths, or CANS, assessment is used to help determine OhioRISE eligibility. Zen Zone does not independently determine whether a child qualifies for OhioRISE.

What if my child is not enrolled in OhioRISE?

Zen Zone's After-School Day Treatment currently serves youth through OhioRISE. A family may still complete the Interest Form to discuss next steps, but submitting the form does not guarantee OhioRISE enrollment or admission. Summer Therapeutic Camp enrollment and payment requirements should be published separately for each camp season.

Is transportation available?

Accepted Answer

Transportation may be available for certain programs and from select schools or locations. Availability depends on the child's school, dismissal time, program schedule, route, and vehicle capacity. Zen Zone will confirm transportation availability during the eligibility process.

Question 6

Which schools does Zen Zone pick up from?

Accepted Answer

Transportation routes may change based on enrollment and program capacity. Enter your child's current school on the Interest Form, and Zen Zone will let you know whether transportation is available from that location.

Question 7

How long does the intake process take?

Accepted Answer

The timeframe varies. It depends on how quickly intake documents are completed, whether legal-authority documents are needed, assessment availability, program openings, and any required OhioRISE authorization. Completing all requested documents promptly can help prevent delays.

What happens after my child is eligible to move forward?

Accepted Answer

Zen Zone will send the parent or legally authorized guardian intake documents through SimplePractice. After the documents are completed, Zen Zone will schedule an assessment. The assessment helps determine the child's needs, treatment recommendations, and final program placement.

Question 9

Who may consent to services?

Accepted Answer

Consent must generally be provided by a parent or another person legally authorized to make healthcare decisions for the child. Zen Zone may request photo identification and documents such as a custody order, guardianship order, power of attorney, or agency placement documents before services begin.

Are individual and group therapy available now?

Accepted Answer

Not yet. Individual Therapy and Group Therapy are planned future services. Families can join Zen Zone Updates to receive an announcement when enrollment becomes available.

What should I do if my child is experiencing a crisis?

Accepted Answer

Do not use the Interest Form, Referral Form, email, or voicemail to request emergency assistance. If your child is in immediate danger, call 911 or go to the nearest emergency department. For 24/7 mental-health or suicide-crisis support, call or text 988.

After-School Day Treatment

A structured therapeutic program that helps youth strengthen emotional regulation, coping, communication, social, and behavioral skills in a supportive group environment.

Monday-Thursday, 3:30-6:30 PM

Current eligibility and funding requirements are reviewed with each family

Individual Therapy

One-on-one behavioral-health services designed around each youth's individual needs.

Summer Therapeutic Camp

A seasonal therapeutic program that combines structured activities, social-emotional skill development, peer interaction, and behavioral-health support.

Publish the approved ages, dates, hours, cost or funding information, and transportation details for each camp season.

Group Therapy

Therapeutic groups that help youth practice skills and receive support alongside peers.
