Professional Referrals

Use this form to refer a youth for Zen Zone's currently available programs. After reviewing the referral, a Zen Zone team member will contact the parent or legally authorized guardian to discuss preliminary eligibility and next steps.

Submitting a referral does not guarantee admission.

What Happens After a Professional Referral

Zen Zone reviews the referral for preliminary program fit and completeness.

Zen Zone contacts the parent or legally authorized guardian. A professional referral does not replace guardian consent.

If the youth is eligible to move forward, the guardian receives SimplePractice intake documents and completes an assessment.
